Why does pain sometimes continue after an injury has healed?
Pain seems like it should be simple. If something is damaged, it hurts. As the body heals, the pain gradually disappears. Much of the time, that's exactly what happens. But anyone who has lived with persistent pain knows it isn't always that straightforward. Some people continue to experience pain months or even years after an injury has healed. Others have significant changes on an X-ray or MRI with little or no pain at all. Two people can have the same diagnosis yet experience completely different levels of pain.
Over the past few decades, researchers have learned that pain is far more complex than we once thought. Rather than being a direct measure of tissue damage, pain is now understood as a protective experience shaped by many different biological processes. That doesn't mean pain is imagined, or that injuries don't matter. But it does mean that tissue damage is only one part of the story.
Pain is part of the body's protective system
Pain has one primary purpose: to protect us from harm. When you touch something hot, pain encourages you to pull your hand away. If you sprain your ankle, pain encourages you to stop running and give the injured tissues time to heal. In these situations, pain is doing exactly what we would hope it would do. But protection isn't determined by injured tissues alone.
The body is constantly gathering information about what's happening both inside and around us. Signals from our tissues and nerves are only part of that picture. Previous injuries, inflammation, illness, sleep, stress, emotions, attention, and even the environment can all influence how the nervous system responds to a potentially threatening situation. Rather than acting like a simple alarm that switches on whenever damage occurs, pain is better understood as the result of the body weighing up many different sources of information before deciding how much protection is needed. Most of the time, this system works very well. It helps us avoid injury, recover after illness, and protect vulnerable tissues while they heal. Sometimes, however, the system becomes more protective than it needs to be. This is one reason pain can continue even after the original injury has largely recovered.
Why can pain continue after healing?
Most tissues in the body heal within a predictable timeframe. The exact timing depends on the type of tissue and the severity of the injury, but healing generally occurs over weeks to months rather than years. Yet many people continue to experience pain long after this process should have been completed. This can be confusing. If the tissues have largely healed, why does it still hurt? In many cases, pain improves as tissues heal. Sometimes, though, the two begin to follow different paths.
During an injury, the nervous system becomes more alert to help protect the affected area. This is an important part of recovery. The problem is that, in some people, this heightened sensitivity doesn't settle as expected. The nervous system may continue responding as though the body needs more protection than it actually does. Researchers often refer to this as sensitisation. In simple terms, it means the nervous system has become more responsive. Movements, pressure, or other stimuli that were once comfortable may begin to produce pain, even though they are no longer causing ongoing tissue damage.
For some people, pain persists even though an injury has healed. For others, pain occurs alongside a long-term health condition involving ongoing inflammation or disease activity. Conditions such as endometriosis, inflammatory arthritis, or inflammatory bowel disease continue to involve biological processes that are capable of producing pain. Endometriosis, for example, is associated with inflammation, changes in immune function, hormonal influences, and changes in the way the nervous system processes pain. These processes interact with one another and may all contribute to a person's symptoms. Similar interactions occur in many other persistent pain conditions, where ongoing disease activity is only one part of a much more complex picture. Even in these situations, the nervous system remains an important part of the picture. Ongoing disease activity, inflammation, changes within the nervous system, sleep disturbance, stress, movement patterns, and many other biological and psychological factors can all interact to influence how pain is experienced.
This helps explain why two people with the same diagnosis can experience very different levels of pain, and why treatment for persistent pain often involves addressing more than one contributing factor. Persistent pain is rarely explained by one process alone. It reflects the combined activity of many systems working together, and those contributing factors are often different from one person to the next.
Why does pain change from day to day?
One of the things many people with persistent pain notice is that their symptoms aren't the same every day. Pain may be worse after a poor night's sleep, during periods of stress, when they're run down, or after doing either far too much or far too little. Sometimes there is no obvious explanation at all. If pain simply reflected the amount of tissue damage present, these day-to-day changes would be difficult to explain. Instead, they remind us that pain is influenced by many factors working together. Ongoing inflammation, the sensitivity of the nervous system, sleep, stress, mood, physical activity, previous pain experiences, and overall health can all affect how pain is experienced.
This doesn't mean the pain is "psychological," nor does it mean the tissues are no longer important. Rather, it reflects the fact that pain is the result of many biological processes interacting with one another. As those processes change from day to day, pain may change too. This complexity also helps explain why there is rarely a single treatment that works for everyone. Managing persistent pain often involves addressing more than one contributing factor, and the most effective approach may look different from one person to the next.
Is rest always the answer?
Rest plays an important role after many injuries. A fractured bone needs time to heal, or a torn ligament may need protection while the tissues repair. In the early stages of an injury, continuing to push through significant pain can sometimes delay recovery or make the injury worse. The role of rest, however, often changes as healing progresses.
For people living with persistent pain, avoiding movement altogether isn't always helpful. Over time, reduced activity can contribute to muscle weakness, joint stiffness, reduced confidence in movement, and a nervous system that may become increasingly protective.
After living with pain for a long time, it's understandable that people begin avoiding movements they associate with pain. Sometimes that caution remains helpful, particularly if movements are placing excessive stress on injured tissues. At other times, however, the fear of making things worse can gradually lead people to become less active than they need to be. This can make returning to everyday activities feel increasingly difficult, even when movement itself is no longer causing harm.
Recovery often involves finding an appropriate level of activity and gradually building from there. The right balance will look different for each person and depends on the underlying condition, the stage of recovery, and many other individual factors. For many people, one of the goals of treatment is not simply to reduce pain, but to help restore confidence in movement and support a gradual return to the activities that matter most to them.
A different way of thinking about pain
For many years, pain was often viewed as a direct signal from injured tissues. Today, we know that the relationship between pain and tissue damage is far more complex. Pain remains one of the body's most important protective systems, but it reflects much more than what's happening in our tissues alone. It is influenced by many biological processes working together, each contributing to the body's assessment of whether protection is needed. Pain cannot always be explained by a single finding, and recovery isn't always as simple as waiting for tissues to heal.
For people living with persistent pain, this broader understanding can be reassuring. Pain that continues beyond the expected healing time doesn't necessarily mean the body is continuing to be damaged. It also helps explain why treatment often involves more than addressing injured tissues alone. Improving sleep, restoring confidence in movement, managing stress, addressing ongoing inflammation where appropriate, and supporting the nervous system may all become part of the picture.
Persistent pain is rarely the result of one process, and recovery rarely depends on one treatment. Understanding pain in this way doesn't make it disappear overnight, but it can help explain why recovery often requires a broader approach than many of us were taught to expect.
